Athletic Training Requirements for Incoming Student-Athletes
Welcome to the Shasta College Athletic Training page! Before participating in any athletic activities, all incoming student-athletes are required to complete the following steps to ensure their health and safety.
�� Required Steps for Athletic Clearance
1. Current Physical Examination
All student athletes must submit a physical completed within the past 3 months. This exam must be performed by one of the following licensed medical providers:
- MD (Doctor of Medicine)
- DO (Doctor of Osteopathic Medicine)
- FNP (Family Nurse Practitioner)
- PA (Physician Assistant)
Note: Physicals from chiropractors or other non-listed providers will not be accepted.

Student-athletes must use the official form provided by the Athletic Training Department. Please download and take this form to your medical provider.

You can upload your completed form to SportsWare once signed by your provider.
2. Create an Athlete Profile in SportsWare
Athletes are required to create a profile and complete required documents in SportsWare Online, our secure medical records system.

To register:
- Visit [insert SportsWare link]
- Click “Join SportsWare”
- Use your school-issued email
- Complete your profile and upload your current physical
Returning athletes should log in and update their information for the current academic year.
3. Concussion Baseline Testing (Sway Medical)
All athletes must complete baseline concussion testing using Sway Medical.
To complete your test:
- Download the Sway Medical app (iOS or Android)
- Use the team or program-specific access code provided by your coach or athletic trainer
- Complete the test following the app's instructions
This test must be completed before participating in any team practice or games.

Haley Mulvihill

Bio: Haley Mulvihill is entering her ninth year as the Head Athletic Trainer at Shasta College, where she leads the sports medicine team in providing comprehensive care for the college’s student-athletes. A certified athletic trainer (ATC), Haley brings a strong foundation of clinical expertise and a deep commitment to athlete health and performance.
Originally from Maui, Hawai‘i, Haley earned her bachelor’s degree in athletic training from Lindenwood University and went on to complete her master’s degree in Kinesiology at Humboldt State University. Her professional experience spans NCAA Division II, NAIA, and community college levels, giving her a well-rounded perspective on athletic healthcare across various levels of competition.
Haley is passionate about injury prevention, rehabilitation, and athlete education. Outside of work, she enjoys spending time outdoors with her family, taking full advantage of nature whenever possible.

Bio: My passion for sports and helping others led me to pursue higher education in health and athletic performance. I began my academic journey at Santa Rosa Junior College, where I earned my EMT certification and an Associate’s Degree in Natural Sciences. While at SRJC, I was a pole vaulter on the track and field team and actively involved with the college’s sports medicine program.
I continued my education at the University of Idaho, where I completed a Bachelor's Degree in Exercise, Sport, and Health Sciences, followed by a Master’s Degree in Athletic Training. Now back in the North State and close to my hometown of Cottonwood, I’m excited to serve the student-athletes at Shasta. I’m passionate about injury prevention, rehabilitation, and supporting athletes as they achieve their goals. In my free time, I enjoy being outdoors and staying active through sports and recreation.
