STUDENT ATHLETE INFORMATION

 

All student-athletes at Shasta College are required to complete a physical specifically related to participation in athletic events before they can take part in any Shasta College related athletic activity.  This examination MUST be recorded on the Shasta College physical examination form. Any physical not completed on this form will not be accepted. An MD (Medical Doctor) or DO (Doctor of Osteopathy) MUST complete this physical. Physicals completed by Physician Assistants, Nurse Practitioners, Physical Therapists, or Chiropractors WILL NOT BE ACCEPTED.

Returning Student Athletes are required to complete a returner form as well as confirm that there is no change to insurance and emergency contact information.

Transferring Student Athletes that would like their Medical Records to be sent to a different school will need to complete the Medical Release Form using the link below.

Mike Davis, Head Athletic Trainer

Mike has over 30 years of athletic training experience.  During his career, he has worked with professional, college, and high school athletes.  Mike did his undergraduate work at California State University Chico and received his Master of Science degree from the University of Arizona.  Mike is certified by the National Athletic Trainers' Association.  His spirit, enthusiasm, and wisdom comes from his wife, Lee and their two children, Cody, and Micaela.

 


 

   

    

Haley Mulvihill, Head Athletic Trainer

Haley received her Bachelor of Science in Athletic Training from Lindenwood University in 2012 where she played water polo competitively. Haley received a Master of Science with an emphasis in Exercise Science from Humboldt State University in 2014. Since becoming a Certified Athletic Trainer Haley has worked in the physical therapy, university and community college setting. Haley brings a diverse range of experience to the Shasta College training staff.
